Trouble configuring different resolutions

Hi, so i have a PB Server Quad with M4000 graphics card. Running driver version 376.84. Windows 8.1 on the machine is fully up to date.
My problem is we cant ever get the machine to run with one output at 3840x2160@25hz(or 50hz) and the other 3 at 1920x1080@50hz. the 4k output will just fall back to 640x480.
should i be able to do this? i would have thought i could do any combination of different sized outputs.
The M4000 specs do say it's capable of 4x 4k outputs.

Yes, you should be able to do that.
Just make sure your refresh rates are all the same over all outputs. (Mean they all need zo be 50 Hz and not 3x 50Hz and 1x 49.89 Hz or something like that.)

The easiest way to accomplish what you want to do is to save EDIDs into the M4000.

What is your EDID Source? What are your devices behind the GPU and how are you connection those?
